Maybe I have misinterpreted the reading of the rule or it is left to the subjectivity of the official, but there sure seems to be intent to injure on that "block".
"Subjectivity of the official" is where I see the answer found. In the tweet Reier sent out about the identical play earlier in the game (not true) you can see the DE in a three point stance, inside of the tackle box.
With Hand, I don't recall seeing his hand ever in the dirt. He looks like he's almost in the JACK role--standing up at the line of scrimmage but outside the box. To me, I'm thinking the official thought he was inside the box even if he was standing up.

To one thing there is no doubt: this type of blocking leads to injuries, often.
